Job summary
Carolina Family Care, Inc. (CFC) is seeking a Nurse Care Coordinator to support the Patient-Centered Medical Home model.
The role collaborates with physicians, staff and care teams to coordinate care for an assigned population; work from home with travel to SC clinics as needed and participation in rounding. Applicants must be a South Carolina-licensed Practical Nurse with at least five years of medical office experience and strong data interpretation, organizational, and communication skills.
Qualifications
- Graduation from an accredited Practical Nurse program.
- Current South Carolina LPN license with at least 3 years of experience as an LPN.
- Proficiency in MS Word, Excel, PowerPoint; EMR experience; strong written and verbal communication.
