Job summary

Carolina Family Care, Inc. (CFC) seeks an experienced Nurse Care Coordinator to support the Patient-Centered Medical Home model in Charleston, SC.

You will collaborate with physicians, staff and providers to facilitate care delivery and enhance care team communication for an assigned patient population. The role requires SC residency, RN licensure, and BLS certification, with 40 hours per week and potential time at MUSC downtown.

Qualifications

• Bachelor’s degree in nursing and RN licensure required.
• Current BLS certification (AHA or Red Cross).
• Three years of related nursing experience.

Responsibilities

• Identifies patients that qualify for care coordination and arranges follow-up services as appropriate.
• Communicates effectively with patient, care team and providers to support continuity of care.
• Identifies patient needs/barriers to care and coordinates contact with community resources.
