Job summary

Carolina Family Care, Inc. (CFC) is seeking a Nurse Care Coordinator to support a PCMH program. This is a primarily work-from-home role with occasional clinic work and required travel within South Carolina.

The coordinator will collaborate with physicians, staff, and care teams to enhance care delivery and communication for a defined patient population. The role requires strong nursing background, proficiency with Epic, and experience in care coordination and quality measures.

Qualifications

• Bachelor’s prepared registered nurse with SC licensure.
• Experience in care coordination and managing CCM/quality measures preferred.
• Strong communication and data documentation skills.

Responsibilities

• Identify patients qualifying for care coordination and follow up on gaps in care.
• Utilize Epic registries to identify patient needs and track quality measures.
• Outreach patients for CCM services and document attempts and outcomes.
• Schedule services and place referrals per patient needs (vaccines, labs, referrals).
• Follow up to ensure continuity of care and data accuracy.
• Communicate with patient, care team, and providers to support continuity of care.
