The Medical University of South Carolina is seeking a Nurse PCMH Care Coordinator to collaborate with physicians and care teams to actively facilitate health care delivery for an assigned patient population. Qualifications

• Graduation from an accredited Practical Nurse program and 5 years of relevant medical office experience required.
• Current South Carolina LPN license required with a minimum three years’ experience as an LPN.
• Proficiency with MS Word, Excel and PowerPoint; working knowledge of an electronic medical record (EMR) system; data interpretation skills.

Responsabilités
