Carolina Family Care, Inc. (CFC) in South Carolina seeks a Nurse PCMH Care Coordinator to collaborate with physicians, staff and other health care professionals to actively facilitate health care delivery and promote care team communication for an assigned patient population.

This is primarily a work-from-home position with travel/rounding and clinic time as needed; applicants must reside in South Carolina near Charleston.

Qualifications

Graduation from an accredited Practical Nurse program and 5 years of relevant medical office experience.
Current South Carolina LPN license with a minimum of three years' experience as an LPN.
Computer skills include MS Word, Excel and PowerPoint.
Working knowledge of an electronic medical record application.
Must be a team player who can follow processes to reach a common goal.
Highly organized with strong oral and written communication skills.
Must be able to analyze and interpret data.
LPN licensure or compact state license is required.

Responsibilities
