Job summary

Pathways Healthcare, a leading home health and hospice organization, seeks a Hospice Liaison to support patients transitioning from home health to palliative or hospice care. The role collaborates with clinical teams, gathers patient information, and maintains strong relationships with referral partners.

You will evaluate patient needs, coordinate with physicians and care teams, and help ensure timely, safe transitions while upholding patient confidentiality.

Qualifications

• Two years of healthcare and/or marketing experience preferred.
• Strong verbal and written communication skills.
• Excellent interpersonal and organizational abilities.
• Ability to work independently with minimal supervision.
• Comfortable learning and using new technology.
• Valid driver’s license and reliable transportation.

Responsibilities

• Build and maintain relationships with home health, palliative, and hospice teams.
• Communicate regularly with case managers and physicians about patient status.
• Meet with patients and families to assess needs and readiness for hospice care.
• Coordinate with physicians and clinical teams to ensure smooth transitions.
• Update internal teams on changes to admission schedules.
• Support the opening and growth of new centers.
• Participate in trainings, meetings, and daily operational briefings.
• Maintain patient confidentiality at all times.
• Report incidents and safety concerns promptly.
• Follow infection control guidelines and safety procedures.
• Listen to and respond to patient and family feedback.
