Residential Home Health and Residential Hospice is seeking a compassionate Hospice Transitional Nurse Liaison to join our care team. The role collaborates with field staff and leaders to evaluate patient needs, coordinate start-of-care visits, and ensure seamless transition to hospice services.

You will conduct assessments, collect medical histories, order DME, educate patients and families, and support discharge planning with facility teams to prevent readmissions.

Qualificações

• Associate degree in Nursing required.
• Active RN license in good standing.
• Minimum 2 years RN experience incl. acute care or oncology; hospice preferred.
• Proficiency with Microsoft Office; familiarity with EPIC and HCHB preferred.

Responsabilidades

• Perform comprehensive patient and family assessments to determine hospice needs.
• Collect accurate demographic and medical history information.
