Job summary

Enhabit Home Health & Hospice is seeking a field-based professional to represent the branch in external transitional care activities and with patients, families, and health systems. You will develop patient-centered transition plans using clinical guidelines to promote quality and efficiency.

The role requires licensure in the practicing state and at least one year in home care, plus basic tech skills. A strong understanding of federal, state, and local hospice regulations is essential.

Qualifications

• Must be graduate of an approved school of clinical education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience in home care.
• Must have demonstrated knowledge of federal, state, and local hospice laws and guidelines.
• Must have basic technology skills, including a mobile device.
• Demonstrated hospice knowledge is preferred.

Responsibilities

• Represent the branch in external transitional care activities and professional contacts to apprise them of Enhabit Home Health & Hospice services.
• Integrate clinical guidelines, protocols, and metrics to develop patient-centered transition plans that promote quality and efficiency.
• Serve as a public awareness representative and educate about provided services.
