Job summary

Telos Health Systems seeks a Telemedicine & Behavioral Health Provider for skilled nursing and post-acute patients. This part-time, fully remote role offers day and night shifts, supporting care across telemedicine platforms with strong clinical autonomy.

Ideal candidates hold APRN/PA licenses with PMHNP certification and have experience in geriatric and behavioral health care, delivering comprehensive decisions and timely escalations as needed.

Qualifications

Active license (APRN/PA) with PMHNP certification or equivalent.
Experience in telemedicine and geriatric/post-acute care settings.
Strong clinical decision-making and independent work style.

Responsibilities

Respond to urgent and routine telemedicine calls from SNF facilities.
Conduct scheduled medical/behavioral health visits and follow-ups.
Evaluate medical and psychiatric concerns via telemedicine and provide treatment plans.
Collaborate with nursing staff and telehealth leadership; document encounters in EHR.
Determine need for higher level of care and coordinate escalations.
