Job summary

NuvemRx is seeking a Patient Referral Coordinator for the NuNavigator Program. This fully remote role supports FQHC partner referrals, appointment scheduling, reminders, and care coordination across sites.

Strong HIPAA compliance and communication skills are required, with preferred bilingual English/Spanish abilities. Ideal candidates have 2+ years in healthcare settings, experience with EHR systems, and a patient-first approach to ensure timely care and smooth information flow between health

Qualifications

• 2+ years in healthcare, medical office, pharmacy, or patient services setting; scheduling, referral coordination, or intake experience preferred.

Responsibilities

• Schedule specialist appointments and confirm details with patients and offices.
• Deliver appointment reminders via phone, text, and other channels.
• Manage no-shows and cancellations; reschedule promptly and keep process moving.
• Maintain waitlists and fill earlier openings when available.
• Schedule follow-up appointments and ensure completion of recommended care.
