Qualifications

• Associate’s degree or equivalent with 3 years of patient relations experience.
• Or a high school diploma/GED with 5 years of related experience in customer relations.
• Strong communication, analytical and problem-solving skills.

Responsibilities

• Serve as a liaison to patients with internal and external care providers and processes of care within their area and across the enterprise.
• Proactively round to meet new patients and promote available programs and services.
• Document all contacts.
• Remove barriers to care and facilitate coordination of care.
• Identify, evaluate, and resolve patient complaints.
• Travel to SNFs and Cleveland Clinic locations as needed.
