The role serves as a key link between patients, providers, and external healthcare entities, handling front desk operations, referral follow-ups, preventive outreach, transportation, and accurate data documentation.

Qualifications

• Bachelor's degree in Social Work or 1+ year experience as CDCA, LCDC I or II.
• Must have a valid driver's license and must pass a background check and drug test.

Responsibilities

• Front desk and patient access operations.
