The Cleveland Clinic Main Campus seeks a Care Coordinator to work with a multidisciplinary team managing high-risk patients across the care continuum. You will coordinate care, perform comprehensive assessments, and promote outcomes and wellness while reducing preventable ED visits and readmissions.

The role emphasizes patient education, advocacy, and seamless transitions of care, with one work-from-home day after orientation and a standard 8:00am–4:30pm schedule.

Qualifikationen

• Graduate from an accredited school of Professional Nursing.
• Current state licensure as a Registered Nurse (RN).
• Basic Life Support (BLS) Certification through the American Heart Association or American Red Cross.
• Three to five years of nursing experience.
• Any RN or APRN must obtain cancer-specific certification or ongoing qualifying education within accreditation cycle if working in oncology within Cleveland Clinic facilities.

• Collaborate with multidisciplinary care teams across the continuum to develop care plans for high-risk patients.
• Coordinate care and disease management longitudinally for patients with chronic conditions.
• Identify patients needing ongoing coordination and conduct targeted outreach.
• Perform comprehensive clinical assessments covering disease, age, medical, behavioral, pharmacological, social, and end-of-life needs.
• Educate patients and families, monitor adherence to care plans, and update plans as needed.
• Act as liaison and advocate for patients and families across transitions of care.
