Qualifications

Responsibilities

• Educate potential patients and family members about the program and enroll patients.
• Assist patients in navigating the program app/website and resolve technical issues.
• Initiate outbound calls and respond to inbound inquiries promptly.
• Assess patient eligibility using provided documentation and address questions.
• Record accurate information and call notes in our system (insurance, provider, clinical updates).
• Aim to achieve weekly productivity targets.
• Provide timely follow-up to interested patients and deliver excellent customer service.
• Stay informed on cardiac rehabilitation developments and participate in training sessions.
• Send educational material to interested patients via email and other channels.
• Communicate insurance or other information with relevant parties as needed.
