Responsibilities

• Monitor therapy outcomes, adherence, and side effects.
• Conduct medication reconciliations and reviews for high-risk patients.
• Collaborate with physicians, nurses, and case managers to coordinate treatment plans.
• Document interventions and communications per standards.
