The ideal candidate has a PharmD or Bachelor of Pharmacy and an active U.S. pharmacist license; ambulatory care or clinical experience is preferred. Flexible scheduling and remote work enable comprehensive patient care from a home setting.

Qualifications

• PharmD or Bachelor of Pharmacy degree required.
• Active U.S. pharmacist license; multiple state licenses preferred but not required.
• Ambulatory care/MTM/clinical experience preferred; hospital and community pharmacists encouraged to apply.
• Strong communication and ability to build rapport with patients virtually.

Responsibilities

• Deliver telephonic or video-based medication counseling and education to patients and caregivers.
• Monitor therapy outcomes, adherence, and side effects; recommend optimizations as needed.
• Conduct medication reconciliations and reviews, especially for high-risk or complex patients.
• Collaborate with physicians, nurses, and case managers to coordinate treatment plans.
• Document all interventions and communications according to organizational standards.
• Support population-health and disease-management programs.
