Job summary

Pharmacy Careers is seeking a Remote Patient Care Pharmacist to provide telephonic or video-based counseling, MTM and adherence support from home. This role suits pharmacists who enjoy patient interaction and flexible scheduling while delivering high-quality care.

Qualifications

Strong communication and interpersonal skills for virtual care.
Empathy and the ability to build rapport with patients remotely.
Experience in ambulatory care, MTM, or clinical pharmacy is preferred.

Responsibilities

Deliver telephonic or video-based medication counseling and education to patients and caregivers.
Monitor therapy outcomes, adherence, and side effects; recommend optimizations as needed.
Conduct medication reconciliations and reviews for high-risk or complex patients.
Collaborate with physicians, nurses, and case managers to coordinate treatment plans.
Document all interventions and communications according to organizational standards.
Support population-health and disease-management programs.
