Talantir seeks a Patient Care Coordinator to support patients through their healthcare journey from a virtual setting. You will engage by phone and digital channels to assess needs, provide education, and connect patients with clinicians and community resources.

You will document encounters in EHRs, schedule appointments, remind patients, and assist with medical records and insurance coordination while maintaining HIPAA privacy and high-quality care.

Qualificações

• Active CMA/CCMA/RMA certification is required.
• Residence in the continental US required.
• Minimum of four years of clinical experience.
• Experience in care coordination, health coaching, and motivational interviewing.

Responsabilidades

• Provide patient care coordination via phone and digital channels.
• Document encounters in EHR systems.
• Coordinate with pharmacists, nutritionists, nurses, social workers, and doctors.
• Schedule appointments and send reminders; follow up as needed.
• Assist with medical records retrieval and insurance coordination.
