Nufactor is seeking a Patient Care Coordinator to act as liaison between patients, nursing partners, and internal departments. This role supports ongoing patient care, updates, and medication coordination for home administration.

You will handle shipping issues, maintain patient census, and ensure compliant processes while collaborating across teams. The position offers remote work with a 40-hour week, requiring strong communication, organization, and a patient-centered approach to service

Qualifications

• High school diploma or equivalent required; college degree preferred in nursing, health sciences, or related field.
• Experience with Microsoft Office and Excel for patient data management and reporting.
• Strong organizational and communication skills with ability to work cross-functionally.

Responsibilities

• Coordinate patient care and serve as liaison between patients, nursing partners, and departments.
• Provide high-level patient service coordination and support to the Patient Care Services team.
• Maintain patient census and manage updates, orders, and medication coordination for home administration.
• Assist with shipping issues and ensure compliance with company policies and regulatory requirements.
