Job summary

Form Health is a national telemedicine weight loss clinic seeking a Medical Assistant-certified PCC to support physicians, dietitians, and patients. You will coordinate care teams, manage messaging, and assist with labs, records, and billing in a fully remote environment.

The role emphasizes patient-centered communication, organization, and collaboration with clinicians to ensure smooth care delivery and program success.

Qualifications

• Medical Assistant certification required (CMA, RMA, or CCMA).
• At least two years in a fast-paced primary care/obesity medicine/endocrinology setting.
• Experience with prescription prior authorization is a plus.
• Excellent verbal and written communication skills.
• Very organized with the ability to manage multiple tasks and adapt to IT systems.

Responsibilities

• Serve as the primary contact to maintain communication between patients and care team.
• Triage and relay messages between patients and clinicians; support with prescriptions and labs.
• Provide scheduling, billing, and benefits coordination support for members.
• Assist with pre-visit labs, medical records review, and data collection for quality metrics.
