Iconic Care Support Services is seeking a Remote Medical Assistant to support our Principal Care Management (PCM) program. The role focuses on ongoing care coordination, education, and patient support between provider visits, in a fully remote setup for WA residents.

The ideal candidate has a Medical Assistant Certification and at least 5 years of experience in medical offices or chronic care management, with strong EHR skills and excellent communication abilities.

Qualifications

• Medical Assistant Certification required.
• Minimum 5 years in medical office, care coordination, or chronic care management.
• Strong computer skills and proficiency with EHR.
• Excellent verbal and written communication skills.
• Ability to work independently in a remote setting.
• Knowledge of HIPAA regulations and patient privacy requirements.

Responsibilities

• Coordinate care for patients with complex chronic conditions within the PCM program.
• Educate patients and coordinate with providers and caregivers.
• Document interactions and care plans in the EHR system.
• Provide remote support and consultations as part of care management.
• Maintain HIPAA-compliant workflows and high-quality patient service.
