Mosaic Health is seeking a Patient Care Specialist II to join our Home Health team in a remote role. You will coordinate front office operations, scheduling, patient communications, and documentation to support efficient care center operations and patient access.

The position requires 2+ years in healthcare, knowledge of EHR systems and insurance verification, and strong communication with patients and families.

Qualifications

• High school diploma or GED.
• 2+ years in healthcare or patient access.
• Strong knowledge of EHR systems, insurance verification, and medical terminology.
• Attention to detail and interpersonal communication.

Responsibilities

• Coordinate patient scheduling, registration, visit preparation, and follow-up across care center workflows.
• Verify patient demographics, insurance information, referrals, and required documentation.
• Respond to patient calls, messages, and front desk inquiries.
• Identify scheduling gaps and resolve routine issues.
• Route clinical messages, refill requests, forms, and care-related inquiries.
• Maintain accurate records in EMR, scheduling, and patient communications systems.
• Support daily front office workflow by coordinating with clinical staff and escalating issues.
