Qualifications

• Experience in a medical office or healthcare coordination.
• Strong communication and documentation skills.
• Proficiency with EHR and basic computer applications.

Responsibilities

• Conduct monthly outreach to assess care needs for ~200 CCM patients.
• Document CCM activities and time in the EHR.
• Support medication reconciliation and adherence follow-up.
• Coordinate appointments, referrals, and follow-up after hospital visits.
• Escalate concerns to the supervising provider as needed.
• Maintain CCM records and billing compliance.
