Job summary

VitalCaring Group in the United States seeks a Hospice Social Worker to support patients and families facing terminal illness with emotional support, resource coordination, and crisis intervention within a compassionate, values-driven team.

You will collaborate with the interdisciplinary team, develop care plans, assist with advance directives, and connect families to community resources, ensuring dignity, comfort, and holistically guided care.

Qualifications

• Master’s degree in Social Work (MSW) required.
• Experience in hospice social work preferred.
• Strong counseling and interpersonal skills.
• Must be comfortable performing home visits with reliable transportation.

Responsibilities

• Assess psychosocial status of patients and families; communicate with RN and IDG.
• Provide counseling for stress, grief, and coping; crisis intervention when needed.
• Coordinate resources, referrals and funeral planning support.
• Educate team on cultural factors influencing care and advance directives.
• Develop individualized plans of care with patient and family.
• Document progress notes and act as liaison with community agencies.
