Job summary

Devoted Health in the United States is seeking a Palliative Care Social Work Case Manager to support our virtual program designed for members with complex, life-limiting illness. You will advocate, coach, and connect patients and families with resources, aligning clinical and community supports to improve quality of life.

You will lead conversations on goals of care, provide supportive counseling, and coordinate with a multidisciplinary team while utilizing telehealth, EHRs, and care planning

Qualifications

• Master's degree in social work and active license required.
• Minimum 2 years of palliative or hospice experience with goals of care conversations and advanced care planning.
• Certified hospice and palliative social work credential, or willingness to obtain within one year.
• Comfortable with technology and fast-paced environment.
• Self-starter with ability to succeed in a work-from-home environment.
• Strong interpersonal and communication skills.
• Comfortable working with patients over video and phone.

Responsibilities

• Telephonic and video visits with patients and caregivers.
• Establish trust and build strong relationships with patients.
• Collaborate with clinicians, nurses, and care coordinators for cohesive, patient-centered care.
• Conduct Social Work interventions to maximize program effectiveness.
• Balance multiple priorities from complex patients.
• Support family members in understanding needs and how to help.
