Devoted Health is seeking a seasoned Palliative Care clinician-leader to provide direct patient care to our enrolled patients via phone or video visits and participate in weekly interdisciplinary team meetings. You will lead clinical staff, drive new workflows, and participate in rapid iteration cycles to improve care delivery.

You will coach and supervise a mix of NP/RN/SW staff, set goals, conduct regular performance reviews, and recruit new team members.

Qualifikationen

• 5+ years of experience as a Palliative Care Clinician.
• 2+ years of experience leading frontline clinical teams with accountability for team outcomes.
• Ability to champion new initiatives and drive business outcomes.
• Strong clinical and non-clinical judgement with coaching to improve performance.
• Deep empathy and commitment to patients and families.

Aufgaben

• Provide direct patient care to palliative care patients by phone or video.
• Lead patient panel reviews and participate in weekly IDT meetings.
• Direct supervisor of palliative care clinical staff (NP/RN/SW) with coaching and performance management.
• Recruitment, interviewing, and onboarding of new clinical staff.
• Conduct clinical audits to maintain QA and promote a member-centric culture.
