Tuesday Health, LLC is seeking a Palliative Care Licensed Clinical Social Worker (EST) to join a remote team serving patients with serious illness. You will collaborate with an interdisciplinary clinical team to deliver psychosocial support, advance care planning, and goal-aligned care across the continuum.

Responsibilities include conducting psychosocial assessments, facilitating conversations, documenting care plans, and guiding care transitions, all within a value-based care model.

Qualifications

• Master’s in Social Work (MSW) from an accredited program.
• Active, unrestricted Licensed Clinical Social Worker (LCSW) license in the applicable state(s).
• Clinical experience in palliative care, including advance care planning and serious illness conversations.
• Experience working within an interdisciplinary care model, collaborating with physicians, advanced practice providers, nurses, care managers, health plan partners, and community-based resources.
• Demonstrated ability to exercise independent clinical judgment, including knowing when to expedite concerns or respond in urgent situations.
• Strong verbal and written communication skills with the ability to document clinical assessments, interventions, and care plans.
• Excellent organizational and time-management skills managing multiple priorities while maintaining a high-quality patient and family experience.
• Proficiency in using EHRs, clinical documentation systems, and digital tools, with adaptability to new technologies.
• Ability to work effectively in a fast-paced, evolving environment with flexibility and resilience.
• Commitment to delivering compassionate, patient-centered care aligned with value-based care.

Responsabilités

• Conduct comprehensive assessments evaluating psychosocial distress, mental health needs, caregiver burden, social determinants of health, and risk factors.
• Clinically interpret assessment results to identify priority needs, risks, and interventions.
• Conduct serious illness conversations with patients and families and facilitate advance care planning discussions and documentation.
• Provide clinical recommendations and escalation to physicians and other clinicians per escalation protocols.
• Develop and document interventions within individualized care plans aligned with patient goals and values.
• Participate in interdisciplinary rounds and case conferences to support coordinated care and patient-family experience.
• Deliver short-term clinical counseling, psychoeducation, and emotional support within the scope of LCSW licensure.
• Identify psychosocial barriers, recommend evidence-based interventions, and support transitions across care settings.
• Document assessments and care plans using electronic systems for coordinated care and quality measurement.
