Job summary

Tuesday Health is seeking a licensed clinical social worker (LCSW) to join a remote-based palliative care team, with a requirement to reside in Eastern Time. The role focuses on psychosocial assessment, care planning, and family support within a value-based care model.

The LCSW will collaborate with an interdisciplinary team, conduct serious illness conversations, assist with advance care planning, and document interventions in our EHR system.

Qualifications

• MSW from an accredited program.
• Active, unrestricted LCSW license in the applicable state(s).
• Clinical experience in palliative care with advance care planning and serious illness conversations.
• Experience in an interdisciplinary care model.

Responsibilities

• Conduct psychosocial assessments evaluating distress, mental health needs, caregiver burden, and social determinants.
• Interpret assessments to identify priority needs and interventions within palliative care.
• Conduct serious illness conversations and facilitate advance care planning and documentation.
• Provide clinical recommendations and escalation per protocols.
• Develop care plans aligned with patient goals and values.
• Participate in interdisciplinary rounds and case conferences.
• Deliver short-term counseling and psychoeducation to patients and caregivers.
• Identify psychosocial barriers and support transitions across care settings.
• Document assessments, interventions, and care plans in EHR.
• Establish therapeutic relationships grounded in empathy and cultural responsiveness.
