Job summary

Tuesday Health is seeking a Palliative Care Licensed Clinical Social Worker (LCSW) to join our remote, EST-focused team. The role involves addressing psychosocial and practical needs, supporting advance care planning, and collaborating with an interdisciplinary care team to improve quality of life for patients with serious illness.

The LCSW will conduct assessments, lead conversations, develop care plans, and document interventions within a value-based care framework.

Qualifications

• Master’s in Social Work (MSW) from an accredited program.
• Active, unrestricted Licensed Clinical Social Worker (LCSW) license in the applicable state(s).
• Clinical experience in palliative care, including advance care planning and serious illness conversations.
• Experience working within an interdisciplinary care model with physicians, nurses, care managers, and other clinicians.
• Demonstrated independent clinical judgment, including escalation when needed.
• Strong written and verbal communication; documenting assessments and care plans.

Responsibilities

• Conduct comprehensive psychosocial assessments and evaluate distress, mental health needs, caregiver burden, and SDOH.
• Interpret assessments to identify priority needs and interventions within the palliative care model.
• Lead serious illness and advance care planning conversations with patients and families.
• Provide clinical recommendations and escalate to physicians per protocols.
• Develop care plans aligned with patient goals and values.
• Participate in rounds and case conferences to support care coordination.
• Deliver short-term counseling and psychoeducation to patients and caregivers.
• Identify psychosocial barriers and support transitions across care settings.
• Document assessments and interventions using EHR and clinical tools.
