Tuesday Health, LLC is seeking a Palliative Care Advanced Practice Provider to lead clinical delivery in a remote Massachusetts role. You will work with senior leaders and clinicians to ensure high-quality supportive care for patients and families.

Ideal candidates hold an MSN or PA degree, 5 years in palliative/hospice care, MA RN/PA license, board certification, and an active DEA license. The position emphasizes collaboration and value-based care across the network.

Qualifications

• > MSNs and PA studies required; advanced practice credentials preferred.
• < Five years of clinical experience in palliative care, hospice, or serious illness care.
• < Active MA RN or PA license without board actions.
• < Board certification as an advanced practice nurse.
• < Active unrestrictive DEA license.
• < Eligible for malpractice coverage.
• < Experience with value-based contracting and network coordination.
• < Proven leadership and collaboration abilities.

Responsibilities

• Lead internal and joint case rounds to ensure barrier-free care and appropriate care plans.
• Coordinate with network and longitudinal care providers to align services for members.
• Review population-level data to identify trends and implement interventions in the clinical model.
• Support provider and clinical staff training to share up-to-date knowledge across the team.
• Support quality management and quality improvement initiatives.
• Provide feedback and guidance to network partners to ensure performance and accountability.
• Collaborate with clinical, operational, financial, and technology teams for seamless integration of services.
• Monitor network data, outcomes, satisfaction, and cost-effectiveness to identify growth opportunities.
• Represent the company in external forums related to palliative care and network management.
• Stay current with trends and regulations in palliative care and network management.
