Job summary

Baystate Health is seeking an experienced Advanced Practitioner to join the Palliative Care division in a community-based role. The position emphasizes symptom management for advanced illness, close collaboration with Social Work, Nursing, and Physicians, and participation in a telehealth-enabled care model.

The role requires BC Nurse Practitioner or Physician Assistant credentials with relevant home care or geriatrics experience.

Qualifications

• BC NP or PA with clinical experience in home care, palliative care, or geriatrics.
• Ability to hold family meetings and discuss goals of care with patients and families.
• Experience working with an interdisciplinary care team and coordinating across disciplines.

Responsibilities

• Assess and manage symptoms of patients with advanced illness in a consultative model.
• Collaborate with inpatient palliative care team and other specialists.
• Participate in interdisciplinary team meetings to support professional development.
• Provide care via telehealth and in-person visits as part of a community-based program.
