Ideal candidates hold MSN or PA credentials, have 5+ years in palliative care or hospice, and maintain an Ohio license. This remote-friendly role offers collaboration with cross-functional teams and meaningful impact.

Qualifications

• MSN or PA degree and active Ohio license with no board actions.
• Board certification as an advanced practice clinician.
• Minimum 5 years in palliative care, hospice, or serious illness care.

Responsibilities

• Lead internal and joint case rounds to ensure care plans are appropriate.
• Coordinate with network providers to optimize services for members.
• Review population data and implement data-driven interventions.
• Support provider and clinical staff training across the team.
• Contribute to quality management and continuous improvement.
• Monitor network data: utilization, outcomes, satisfaction, costs.
• Collaborate with clinical, operational, financial, and tech teams.
• Represent the company in external palliative care forums and events.
