HarmonyCares, a physician-led provider of home-based primary and palliative care, seeks an Advanced Practice Provider to serve medically complex, home-limited patients.

You will conduct in-home and telehealth visits, perform comprehensive assessments, and manage chronic conditions while supporting palliative care needs. Collaborating with an interdisciplinary team, you will coordinate care plans to reduce avoidable hospitalizations and improve outcomes, in a mission-driven, supportive culture.
