Job summary

Tuesday Health is seeking a Licensed Clinical Social Worker (LCSW) to join a value-based palliative care team. You will assess psychosocial needs, support advance care planning, and coordinate with physicians and other clinicians to ensure goal-aligned care for patients at home or in remote settings.

Applicants must have an MSW and an active LCSW license in Texas, with experience in palliative care and comfort using EHRs.

Qualifications

• MSW from an accredited program.
• Active, unrestricted LCSW license in Texas.
• Clinical palliative care experience required.

Responsibilities

• Conduct psychosocial assessments and identify needs for patients with serious illness.
• Lead serious illness conversations and advance care planning.
• Provide counseling and support to patients and families.
• Collaborate with interdisciplinary team to align goals with care plans.
• Document assessments and care plans in EHR systems.
