Job summary

Jobgether partners with a healthcare client to fill a PAC Engagement Specialist role based in the United States. You will strengthen engagement across a network of post-acute care facilities, drive adoption of a clinical intelligence platform, and support care coordination to improve patient outcomes.

You will work remotely within a fast-paced Client Success team, monitor engagement, train users, and collaborate with Network Operations and Training to address challenges and optimize performance.

Qualifications

• 3+ years of experience in provider relations, provider outreach, network management, or care coordination.
• Knowledge of post-acute care providers (SNFs, LTACs, home health, hospice, IRFs).
• Experience with outreach by phone and email, including cold calls, to improve engagement.
• Proficient with Excel (filters, pivot tables, lookup) and Outlook.
• Experience with CRM systems is a plus.

Responsibilities

• Monitor engagement across clients using dashboards and reports.
• Identify facilities with low engagement and develop outreach plans.
• Conduct outbound calls to establish relationships and support user registration.
• Manage email outreach campaigns to encourage facility participation.
• Onboard and train new users on platform functionalities and workflows.
• Maintain accurate facility records and engagement data.
• Escalate issues to appropriate teams and communicate feedback for improvements.
• Prepare engagement reports and monthly updates for stakeholders.
• Contribute to network-wide communications and newsletters.
