Job summary

Aledade PBC is seeking an Outreach Specialist for the ACCESS Team to drive patient participation, enrollment, and activation for the ACCESS program in a remote, patient-centered role. You will support CMS Innovation Center's ACCESS model by guiding onboarding, collecting clinical metrics, and triaging patient needs with high responsiveness.

This frontline position emphasizes outbound/inbound outreach, device onboarding, and rapid triage while maintaining HIPAA compliance and documentation.

Qualifications

• High school diploma or equivalent.
• Minimum of 2 years of experience in healthcare outreach, care coordination, or telephonic patient engagement.
• Proven ability to meet performance metrics in a high-volume environment.

Responsibilities

• Conduct outbound and inbound outreach calls when automated workflows fail or require manual intervention.
• Assist patients with onboarding, clinical metric collection, and device onboarding troubleshooting.
• Deliver engagement nudges to prompt completion of care management tasks and support ongoing care continuity.
