Job summary

Waymark in Chicago is seeking an Outreach & Care Coordinator Manager to lead our market’s outreach and care coordination professionals. Reporting to the VP of Market Operations, you will supervise day-to-day activities, coach the team, and shape the CC experience across recruitment, training, deployment, and support.

You will build partnerships with PCPs and community resources while monitoring performance dashboards and ensuring compliance.

Qualifications

• Deep commitment to improving healthcare for underserved populations.
• History of supervising teams of care coordinators, patient navigators, customer service, outreach specialists, or similar roles.
• Leadership with cultural and personal humility.
• Strong attention to detail and ability to monitor metrics with data analysis.
• Ability to collaborate in a multidisciplinary team.
• Experience with healthcare data and/or Electronic Health Records.
• Willingness to travel within the Chicago area to establish new teams.
• Knowledge of Medicaid/MCOs and barriers to healthcare access.

Responsibilities

• Provide direct supervision of the day-to-day activities of CCs in the market.
• Actively participate in the execution of daily CC tasks alongside team members.
• Lead and support CCs with coaching, mentoring, and support to ensure competency and compliance with workflows.
• Coordinate cross-team workflows with local leaders and care team members.
• Review data dashboards to manage CC performance relative to goals.
• Work with Operations and Learning & Development to hire, train, deploy, and support CCs.
• Foster PCP partnerships through communications and meetings.
• Build community partnerships through in-person or virtual meetings.
• Maintain knowledge of programs and resources available to community members.
• Foster an inclusive team environment and contribute to the local leadership culture.
• This role is mainly remote with some in-person travel required within the market (up to 30%)
