Job summary

Waymark seeks an Outreach Specialist/California Care Coordinator, Manager to lead care coordinators in Sonoma, Orange and San Bernardino counties. The role combines individual contributor duties with team leadership, reporting to Market Operations Lead, and guiding day-to-day priorities, coaching, and performance management.

You will shape the CC experience, including recruitment, training, deployment, mentorship, and advocacy across the organization. Remote with travel up to 30%.

Qualifications

• Experience supervising care coordinators or similar roles.
• Strong understanding of Medicaid/MCOs and barriers to care.
• Detail-oriented with proven metrics monitoring and improvement.
• Ability to collaborate across multidisciplinary teams.
• Experience with healthcare data and/or Electronic Health Records.
• Willingness to travel across local counties.
• Residence in Sonoma, Orange, or San Bernardino counties.

Responsibilities

• Provide direct supervision of CCs in Sonoma, Orange & San Bernardino Counties.
• Participate in daily CC tasks such as patient outreach and care coordination.
• Coach and mentor CCs to ensure competency and compliance.
• Collaborate with local leads to ensure smooth cross-team workflows.
• Review data dashboards to manage CC performance relative to goals.
• Assist with hiring, training, deployment and ongoing support for CCs.
• Foster PCP partnerships through meetings in person or virtually.
• Build community partnerships through in-person or virtual events.
• Maintain knowledge of programs/resources to assist community members.
• Promote inclusive team environment and Waymark values; lead local team culture.
• Role is mainly remote with up to 30% travel within the market.
