Waymark is hiring an Outreach Specialist/Care Coordinator to be the frontline in our region, connecting patients from low-income communities with primary care resources. You’ll work within healthcare facilities and a multidisciplinary care team to refine workflows and build strong relationships with physician partners.

This hybrid role combines remote work with on-site duties, requires travel up to 30%, and involves a paid 3-week onboarding training.

Qualifications

• Excellent verbal and written communication skills.
• Able to tailor communication to physicians and patients empathetically.
• Highly organized and self-motivated to work independently.
• Customer service and/or sales experience.
• Experience with Electronic Health Records.
• Experience in a healthcare delivery setting.
• Passion for working with diverse communities.
• Comfort navigating Google Workspace.
• Residence in Greater Hampton Roads with reliable car; travel up to 30%.
• Must attend a 3-week paid training onboarding.

Responsibilities

• Initiate outreach to new patients and connect to resources via telephonically, SMS, video, or in-person.
• Foster trust and improve patient engagement through empathetic communication.
• Collect, review and analyze patient data from EHRs to prioritize care.
• Communicate with primary care providers through multiple avenues (EHR messaging and in-person).
• Serve as a primary day-to-day contact and build relationships with healthcare facility providers.
• Participate in weekly care team huddles to support coordinated patient care.
• Accurately document each patient encounter in detail.
• Collaborate with support team to test and refine internal workflows.
• Hybrid role involving remote work and facility-based duties.
