Qualifications

• Healthcare experience preferred in patient access, scheduling, or customer service.
• Experience managing high-volume outbound calls or outreach.
• Experience with Epic or similar EHR systems.
• Excellent communication and problem-solving skills.

Responsibilities

• Conduct high-volume outbound patient outreach to schedule AWVs and preventive care.
• Document outreach activities and scheduling outcomes in the EMR.
• Educate patients about preventive care and access options.
• Identify and address barriers to scheduling and engagement.
• Meet productivity, scheduling, and quality goals.
• Collaborate with care teams and internal stakeholders to ensure coordinated care.
