Blue Ridge Cancer Care seeks an experienced Collaborative Practice Nurse (CPN) to support oncology patients across the continuum of care. The CPN acts as a clinical resource, care coordinator, and patient advocate to improve outcomes and patient experience within a value-based care framework.

Responsibilities include symptom assessment, education, navigation of complex needs, and collaboration with physicians.

Formación

• BSN preferred; Oncology Certification or Nurse Navigator certification a plus.
• Minimum 5 years as Registered Nurse; oncology experience preferred.

Responsabilidades

• Respond to patient inquiries via calls, portal messages, and emails, ensuring timely communication.
• Assess symptoms using algorithms and pathways; document interventions.
• Coordinate outpatient visits and lead Patient Care Conferences.
• Support survivorship, care planning, and referrals.
• Participate in research, quality, and performance improvement initiatives.
