Job summary

Qualifications

• Current RN licensure in North Carolina.
• Must reside in the Raleigh metro area or be willing to relocate to Raleigh, NC.
• Experience as an outpatient nurse.
• Strong clinical assessment, patient education, and care coordination skills.
• Ability to manage multiple priorities in a fast-paced ambulatory setting.
• Commitment to WakeMed's patient-centered care model.

Responsibilities

• Provide nursing care management and clinical support for ambulatory patients.
• Triage patient concerns via phone, patient portal, and in-person encounters.
• Collaborate with physicians and care team to develop and implement care plans.
• Identify high-risk patients and coordinate follow-up, referrals, and resources.
• Educate patients/families about conditions, treatments, meds, and preventive care.
• Act as liaison between patients, families, and providers.
• Promote continuity of care and support optimal health outcomes in outpatient settings.
• Maintain accurate documentation and support quality and safety initiatives.
