Overview

WakeMed Physician Practices is seeking an experienced Ambulatory Care Nurse (RN) - OB GYN Triage to join our growing team at WakeMed. While this is a remote position, the right applicant must reside with in the Raleigh metro area or looking to relocate to Raleigh, NC.

The Ambulatory Care Nurse (ACN) plays a vital role in delivering high-quality, patient-centered care by coordinating and managing the healthcare needs of our patient population. Working closely with providers and the interdisciplinary care team, the ACN serves as a clinical resource, patient advocate, and care coordinator, ensuring patients receive timely, compassionate, and effective care throughout

their healthcare journey.

What You'll Do

• Provide nursing care management and clinical support for patients in an ambulatory setting.

• Serve as a communication liaison between patients, families, providers, and the interdisciplinary healthcare team.
• Promote continuity of care and support patients in achieving optimal health outcomes within the outpatient setting.
• Maintain accurate documentation and support quality, safety, and patient experience initiatives.

What We're Looking For:

• Current Registered Nurse (RN) licensure in North Carolina.
• Must reside within the Raleigh metro area or looking to relocate to Raleigh, NC.
• Previous experience working as a nurse in an outpatient setting.
• Strong clinical assessment, patient education, and care coordination skills.
• Ability to manage multiple priorities in a fast-paced ambulatory environment while providing exceptional patient service.
