Job summary

WakeMed is seeking an experienced Ambulatory Care Nurse (RN) for its WakeForest location. The role begins on-site, with a fully remote triage component after 6 weeks of orientation, delivering patient-centered care in an ambulatory setting.

You will coordinate care with physicians and the care team, educate patients, and identify high‑risk individuals to ensure timely follow-up and resources, while documenting thoroughly to support quality and safety initiatives.

Qualifications

• Current RN licensure in North Carolina.
• Experience in an outpatient setting.
• Strong clinical assessment and care coordination skills.
• Ability to manage multiple priorities in a fast-paced ambulatory environment.

Responsibilities

• Provide nursing care management and clinical support for ambulatory patients.
• Triage patient concerns, assess symptoms, and give guidance through phone, portal, and in-person visits.
• Collaborate with physicians and the care team to coordinate individualized care plans.
• Identify high‑risk patients and coordinate referrals and community resources.
• Educate patients and families on conditions, treatment plans, medications, and preventive care.
• Serve as liaison between patients, families, providers, and the care team.
• Promote continuity of care and maintain accurate documentation.
