Qualifications

• Current RN licensure in North Carolina.
• Experience in outpatient settings.
• Strong clinical assessment and patient education skills.
• Ability to manage multiple priorities in a fast-paced ambulatory environment.

Responsibilities

• Triage and manage ambulatory patient care via phone, portal, and in-person encounters.
• Coordinate care with physicians and care team to develop plans of care.
• Identify high-risk patients and arrange follow-up and referrals.
• Educate patients and families about conditions, medications, and prevention.
• Maintain documentation to support quality and patient experience initiatives.
• Serve as liaison between patients, families and the care team.
• Promote continuity of outpatient care and optimal health outcomes.
• Support WakeWay to Excellence and professional nursing standards.
