Job summary

ComplexCare Solutions seeks a Nurse Practitioner for per diem home and telehealth assessments across Lincoln County, Nebraska and beyond. You will conduct comprehensive member reviews, document findings, and coordinate with PCPs and health plans to support safer, independent living.

Active NP license (ANCC/AANP) and CPR required; CMS/HEDIS knowledge a plus. Remote clinical environment with HIPAA compliance and a focus on reducing hospitalizations.

Qualifications

• Active Nurse Practitioner license in good standing.
• ANCC or AANP board certification in Family, Adult, Gerontology, or Emergency Medicine.
• Experience in home health or telehealth preferred.
• Strong communication with patients, families, and care teams.
• Knowledge of CMS/HEDIS guidelines a plus.

Responsibilities

• Conduct comprehensive member assessments in home and telehealth settings.
• Document findings and care plans in the EHR.
• Identify gaps in care and coordinate follow-up with PCPs and health plans.
• Ensure HIPAA compliance and patient confidentiality.
• Provide patient education to improve chronic condition management.
