Job summary

ComplexCare Solutions is seeking a Nurse Practitioner per diem to perform comprehensive home and virtual assessments across Fond du Lac County, WI. The role supports CMS-aligned care management and works to keep members healthy in their homes and out of the hospital.

The position requires an active NP license with ANCC or AANP certification, CHHA-level autonomy in a clinical remote environment, and strong documentation in EHRs. In-home visits and some telehealth may be expected.

Qualifications

• Active unencumbered nursing license.
• Board certification: ANCC or AANP as NP or CNS.
• Excellent communication with elderly and chronically ill patients.
• Ability to practice autonomously in remote clinical environments.
• Knowledge of CMS Regulations and NCQA HEDIS Guidelines.
• CPR certification current.
• Multilingual ability is a plus.

Responsibilities

• Conduct in-home or telehealth assessments to capture patient needs.
• Develop evidence-based treatment plans and document accordingly.
• Coordinate with PCPs and health plans for follow-up and referrals.
• Identify gaps in care and provide recommendations to improve outcomes.
• Maintain HIPAA compliance and accurate medical records.
• Perform Point of Care testing where applicable.
