Job summary

Cadence is seeking a remote Nurse Practitioner to act as the primary clinical contact for patients in a fully remote, Pacific Time–aligned schedule. You will lead GDMT decisions for heart failure, diabetes, and hypertension, with collaboration from physicians, RNs, and care teams to optimize treatment and outcomes.

You must hold an Active Compact Multi-State RN license; NP licensure in CA/WA/AZ is preferred.

Qualifications

• Active Compact Multi-State RN license.
• Active NP licensure preferred (CA/WA/AZ).
• Ability to work Pacific Time hours with some extended coverage.
• 5+ years treating chronic diseases (CHF, HTN, T2D).
• 3+ years cardiology focusing on GDMT.

Responsibilities

• Manage a panel of remote patients and make timely clinical decisions.
• Follow evidence-based protocols and adapt to individual needs.
• Initiate and titrate GDMT with care teams.
• Respond to remote monitoring alerts and escalations.
• Provide synchronous and asynchronous care and schedule visits.
