Job summary

Cadence is seeking a Nurse Practitioner to serve as the primary clinical contact for patients in a remote care journey. You will lead clinical decision-making with a focus on guideline-directed medical therapy for heart failure and adherence to evidence-based guidelines for diabetes and hypertension.

This fully remote role requires high clinical autonomy and collaboration with RNs and patient success teams to deliver scalable remote care while meeting productivity and documentation expectations.

Qualifications

• Active Compact Multi-State RN license
• Active NP licensure in TX
• 5+ years experience treating patients with chronic diseases (CHF, HTN, T2D)
• 3+ years cardiology experience with GDMT focus
• Active ANCC or AANP certification
• Master's Degree as a Nurse Practitioner and willingness to expand state licensure

Responsibilities

• Manage a panel of patients in remote care and make timely clinical decisions based on guidelines for heart failure, hypertension, and type 2 diabetes.
• Deliver evidence-based care by following established protocols and applying clinical judgment.
• Initiate and titrate GDMT in collaboration with physicians and care teams to optimize outcomes.
• Respond to clinical escalations from remote patient monitoring and adapt to evolving workflows.
