Job summary

Mercy in Missouri is seeking a part-time Nurse Navigator to guide patients through their healthcare journey, coordinate care from pre-treatment through follow-up, and help ensure timely, safe, high-quality care in a hybrid work setting.

You will partner with patients, families, physicians, bedside nurses, and interdisciplinary teams to assess needs, connect with resources, educate patients, support discharge planning, and drive quality improvement initiatives.

Qualifications

• Graduate of an accredited School of Nursing.
• Current RN license in the state of practice.
• Minimum of 2-3 years of nursing experience.
• BLS certification.

Responsibilities

• Coordinate care for inpatient and outpatient populations.
• Guide patients through treatment journey; serve as resource and advocate.
• Provide patient and family education, including pre-operative preparation and follow-up care.
• Collaborate with physicians, nurses, and multidisciplinary teams to ensure high-quality, patient-centered care.
• Support discharge planning and coordinate post-acute services.
• Monitor patient progress and help improve outcomes, patient satisfaction, and continuity of care.
• Assist with length of stay (LOS) management and utilization review activities.
• Participate in quality improvement initiatives and implementation of best practices.
• Serve as a clinical resource and mentor to care team members.
• Maintain accurate documentation within the EPIC EMR.
